Patients should obtain a prescription from their physician prior to scheduling an appointment. The prescription should include the patient’s name, a specific diagnosis, and the type of therapy required, as well as the date and physician’s signature. In addition, any restrictions, limitations or precautions should be included. Patients or family members should call 201.418.1164 to schedule an evaluation.
Each patient referred for outpatient rehabilitation will be evaluated by a licensed therapist during his or her first visit. The evaluation will include a functional assessment that will be guided by the patient’s condition and ability. Following the examination, the therapist and the patient will set functional goals with specific time frames. Discontinuation/discharge of rehab services is at the discretion of the licensed therapist in consultation with the referring physician.
