The CarePoint Health Transitional Care Units at Hoboken University Medical Center and Bayonne Medical Center provide a unique, caring environment for those who are not well enough to make a safe transition from the hospital to their home. Our highly skilled staff will help to ensure a safe and successful return home.

The Transitional Care Unit utilizes a multi-disciplinary approach to develop an individualized plan of care for each resident. Our Services
The Transitional Care Units at Hoboken University Medical Center and Bayonne Medical Center have 32 beds combined, offering private and semi-private rooms operating 24 hours a day, seven days a week. The Transitional Care Unit offers physical therapy, skilled nursing, occupational therapy, respiratory therapy and speech therapy along with all the other services of the hospital including:
