Through Neighborhood Health Centers from CarePoint Health, you can receive personalized, continuous and coordinated medical care—regardless of your ability to pay. Whether you’re uninsured,
underinsured or have Medicaid, quality health care is available to you and your family. Medical services are provided by a bilingual staff (Spanish and English).
■ Physicians and nurses who know you. At Neighborhood Health Centers, our medical staff will take the time to get to know you and your family. Each time you visit, you will be seen by a medical professional who understands your medical history and unique concerns. Many of our patients consider us their “medical home,” a place where they feel comfortable and safe while receiving a full range of quality care.

Do you or a family member have a chronic medical condition such as diabetes, high blood pressure or hyperlipidemia? You can manage your condition with care from professionals at a Neighborhood Health Center. Our goal is to help you live the healthiest life possible.
When You’re Ill
Whether you have the flu, a serious cold, bronchitis or another illness, our doctors and nurses are on hand to provide the care you need.
For minor procedures, such as biopsies and joint injections, you don’t have to visit a hospital. We handle many minor procedures at a Neighborhood Health Center.
Neighborhood Health Centers offer an expanding array of specialized care, including:
■ Obstetrics and gynecology
■ Care for HIV patients
Not all services are offered at all three Neighborhood Health Center locations. However, your caregiver can refer you to any location for a specific type of service.
